It's a 3dfx Banshee, i guess with 16mb of SGRAM. Though, those cards usually had SGRAM by EliteMT.

Yes, it's Triple D (Axle) Banshee. This Banshee is quite interesting, because it often uses 7ns RAM modules, which are placed on both PCB sides.
