OK... Colormax Voodoo2:

name: A-MAX -  ColorMAX - VP-510
(A-MAX - ColorMAX are marketing names analogic to e.g. InnoVision - Inno 3D or GainWard - CardExpert etc.)

Both 8/12MB were available, standard frequencies (90/90MHz)

original drivers are available here:

only one picture (I know about):

it seems, that the chips are cooled by 3 passive cooler (A-MAX/ColorMAX Voodoo1 featured similar coolers).

PCB design is not the first one (2 chips near RAMDAC)

It is a voodoo graphics and the memory speed is 35ns.
