Did you try an other glide2x.dll ?

..yes,I have tried other glide2x,even the glide2xto3x wraper,but nothing helps..[:(]

.if I run a glide2x application (unreal tournament,deus ex,incubation,you name it),it works fine,but when I exit to desktop,everything is corrupted (icons are black,text in the popup menus is missing) and I have to restart.really annoying.no such problem with geforces,this problem is radeon exclusive[V]

Did you try to remove the drivers and install them again ?
- remove ATI drivers and their utilities + the Voodoo drivers
- then install again the Voodoo Drivers
- and install again the Ati drivers
