I think daytona never been manufactured with 16Mb memory..
I think this was the plan:
Voodoo 4 4000: VSA-100, 16mb SDRAM
Voodoo 4 4200: VSA-101 (Daytona), 16mb DDR-SDRAM
Voodoo 4 4500: VSA-100, 32mb SDRAM
Voodoo 4 4800: VSA-101, 32mb DDR-SDRAM
My opinion:
Voodoo 4 - 4000: VSA 100; 16MB 128bit SDR SDRAM
Voodoo 4 - 4200: VSA 101; 64MB 64bit DDR/SDR SDRAM
Voodoo 4 - 4500: VSA 100; 32MB SDR SDRAM
Voodoo 4 - 4800: VSA 100; 32MB SDR SDRAM (="V4-4500 + TV-out & DVI")
But this is:
VSA 10?; 16MB SGRAM (SDR or DDR?)
a) VSA 100, 16MB 128bit SDR SGRAM, lower clock (heatsink only)
b) VSA 101, 16MB 64bit SDR SGRAM, higher clock (daytona @ 166MHz requires no heatsink)
c) VSA 101, 16MB 64bit DDR SGRAM, ... -"-
any other possibility?

I have one extension to your list :
My Daytona is a Voodoo4-2 4800, VSA101, 32 MB DDR 64bit + TV-Out
Can you show (/send) me a picture, pls.?

what i think is strange!!!
It's an IBM-OEM-card... It seems it actually was mass-produced!

I newer thought that the Daytona (Voodoo4 4200/4800) was produced!
Since I got one of those now, some close-up Images :

It certainly looks quite different from this only other Voodoo4-2 4200 I've seen so far, and (unfortunately) I have been unable to get it to work with any Driver I've thrown at it (yet, a few are still left for testing)...
Good luck on finding a driver for it FalconFly. I'm going to try and get one myself since it seems this guy has quite a few. :D

The win 2k drivers are busted, looks like they were all build on the same source.
During driver load the driver modifies DRAMInit 0 and 1, Daytona DRamInits are not compatible with VSA100. The BIOS sets them up correctly and the driver nukes them, you end up with a scrambled screen. :(

Is it possible RAMPAGE driver to test he the DDR RAM under supports there?
@Amigamerlin times a good tip of you! please.....
