Could anyone please tell me how I would be able to spot a AGP Voodoo 5 5500 which has AGP 4X compared to one which uses AGP 1X? Are there certain revision numbers I should look for or any pin differences?

The pcb is different simply.
Look how is a V4 4500 Agp (compatible Agp 4x) and you will see that the notch is different.

But remember that it's not 4x AGP, it's 1.5V Agp. You should not run this card at agp 4x.

I am a bit ignorant in this topic but why may I ask should it not run in 4X AGP? and if not 4X then what about 2X?

You don't get any performance gain, that's why. The Voodoo 5 is an AGP 1x part, it's not designed to get any performance gain from running at higher AGP rates, and in practice it actually loses a little performance (at least in my tests). I couldn't even get my V5 to run at 4x in my MSI K7T266Pro2-a, it kept reverting back to 2x with every driver set I tried. Also, just to catch you up to speed, the normal V5 has 1 notch in the AGP connector, a 1.5V V5 has two notches.
